Defense Secretary William S. Cohen has concluded that there were no failures of intelligence or acts of negligence that led to the Oct. 12 bombing of the USS Cole, but that the entire military chain of command, beginning with himself, failed to ask sufficiently probing questions about how to protect U.S. forces against a changing terrorist threat, a senior Defense Department official said last night.
